An opportunity for the public to to learn about the Reaching For Health Initiative; a collaborative, research-based and third sector-led action plan.
Guest speakers introduced the logic behind, and the processes at play for this self-sustaining health impact model to support communities in maintaining healthier lives – ultimately reducing demand upon GP and NHS services over the longer-term.
Discussed how in-community pre-habilitation, rehabilitation and active healthy ageing are the three central pillars upon which the Reaching For Health initiative rolls out. All within regional clusters of modulised, pre-fitted centres which are built, opened and self-sufficient within 18 months – all based upon the proven the Reach For Health Centre model.
Addressed how public procurement and the Social Value Act 2012 underpins this initiative, where NHS suppliers are helped to deliver and uniquely evidence their contractual social value through the impact this initiative delivers.
